
Our hospice services are available in the home,
hospital, nursing home, residential care facility, and any other place patients
call home. Services include pain management, nursing care, nutritional
counseling, social work, spiritual care, physical and occupational therapies as
needed, and bereavement counseling. Together these services create an
environment in which patients and families can share their thoughts, hopes and
concerns.
The hospice team is committed to supporting patient and
family goals, and to encouraging family members to be involved in all care
decisions. The care team includes the patient’s primary care physician, registered nurses, home
health aides, social workers, volunteers, dietitians, chaplains, bereavement coordinators, and the
hospice medical director. The Visiting Nurse hospice team can
be reached 24 hours a day, 7 days a week.
The focus is on quality of life by helping
patients and families with personal choices, providing meaningful experiences and preserving patient
dignity.
